For this assignment, students must firstly write three short (approximately 500-word) patches to demonstrate their achievement of all the following learning outcomes:

Learning Outcome One:

Explain the use of self-management strategies, to include symptom recognition, treatment, self-monitoring, educational interventions, lifestyle modifications – [500 words]

Learning Outcome Two:

Review actions, interventions and responses that care providers should make in response to a deterioration in patient\’s illness or end of life care – [500 words]

Learning Outcome Three:

Explore the role of digital and information technology in supporting individuals/families/carers within a range of health care contexts (e.g. NHS 24, patient information website sites and informatics, health apps, social media etc.) – [500 words]

In addition to the 1500 words detailed above, students must also:

Demonstrate the relevance of their chosen patches, to Anticipatory and Alternative Approaches to Adult Nursing, by writing a final, patch(1000 words approximately) which meets both of the following learning outcomes:

Learning Outcome Four (a):

Discuss the philosophy, theory and underpinning processes of anticipatory care, and their impact on health care delivery

Learning Outcome Four (b):

Describe how nursing models may support holistic assessment and care planning in anticipatory care
